Sacramento has two entirely different drainage systems and which one you are on depends on how old your neighbourhood is. Roughly 7,545 acres of the Central City run on a combined sewer, a single pipe that carries household waste and storm runoff together. In heavy weather, when every capacity in that system is exceeded, it overflows. That is not a scandal, it is nineteenth century engineering still doing its job, and it changes what a backup means at your address.

Two Rivers, Two Plants and a Set of Wells
Sacramento sits at a confluence, and it drinks from both sides of it. The Sacramento River Water Treatment Plant draws from the Sacramento River and the E.A. Fairbairn Water Treatment Plant draws from the American River. Between them the city describes a combined reliable water supply and treatment capacity of 260 million gallons per day, serving a service area of roughly 100 square miles and nearly half a million customers.
Underneath that surface supply sits a set of groundwater wells, given as 32 in the city planning documents. Those wells matter more than their number suggests, because groundwater and river water are chemically different and the parts of the city that lean on wells can behave differently from the parts that run on treated river water.
Surface water is also seasonal in a way groundwater is not. Rivers fed by Sierra runoff change through the year in temperature, turbidity and mineral content, which is why the city runs a full treatment train rather than a simple disinfection step and why taste complaints tend to cluster in particular months rather than spread evenly.
The practical consequence for a homeowner is that Sacramento water is not one fixed thing. Equipment sized against an annual average can behave differently in different seasons, and a symptom that appears every spring and vanishes every autumn is telling you about the river rather than about your house.
The Combined Sewer System, and What It Means at Your House
City documents put approximately 11,300 acres inside Sacramento as contributing flows to the combined sewer system, with roughly 7,500 of those acres in the Central City and the older neighbourhoods around it. The general plan background report gives the Central City figure as 7,545 acres. Downtown, East Sacramento, Oak Park and Land Park are the areas usually named.
A combined sewer does two jobs with one pipe. In dry weather that is efficient and entirely unremarkable. In a serious storm the rainfall arriving in the same pipe as the sewage can push flows past what the system can handle, and the city states plainly that during heavy storms flows can exceed treatment capacity, resulting in combined sewer overflows when all system capacities are exceeded.
For a homeowner inside that footprint there are three real consequences. A backup during a downpour is a different event from a backup on a dry Tuesday and should be described that way when you call. Anything that adds clean water to the sewer, from a sump pumping into a floor drain to a downspout plumbed into a house drain, is making a citywide problem worse and may not be permitted. And a backwater valve is a more serious conversation here than in a separated system.
The rest of the city is on a separated system, where storm drainage and sewage travel in different pipes. If you have moved from one part of Sacramento to another, do not assume the rules that applied at the old address apply at the new one. Never connect a downspout, sump or yard drain into a house drain in the combined sewer area. It is the single easiest way for a homeowner to turn their own roof into part of the overflow problem, and it converts every heavy rain into a private drainage event as well as a public one.

Sacramento is one of the few places where a plumber genuinely has to ask which agency owns the pipe before answering a question about a backup. Four different bodies can be involved depending on the address, and they do not share a boundary with the city limits.

Why does this matter to a homeowner with a slow drain? Because the moment a problem moves past your property line, the question of who to call and who pays is decided by which of these four you are dealing with. Getting that wrong costs days, and days matter when the alternative to a resolved blockage is a flooded floor.
It matters to contractors too. Connection standards, backwater valve expectations and what may be discharged are not uniform across these agencies. Anyone quoting sewer work in this region should be able to tell you which agency governs the address before they quote, not after. There is a third party in the picture that homeowners routinely forget: themselves. The private side lateral, the pipe running from the house out to the public main, is almost always the property owner responsibility whichever agency owns what it connects to. Most of the sewer problems people describe as the city failing are in fact failures on that private run, and the camera does not care whose name is on the pipe.
The way to settle all of this in advance is a five minute job rather than an emergency one. Find out which system serves your address, whether the lateral has ever been surveyed, and where your clean out is. A household that knows those three things loses hours in a backup rather than days.
The House and the Ground Under It
Flat Ground, Old Houses and the Metering Change
Sacramento is flat. That sounds like good news and mostly it is, but flat ground means shallow fall on long drain runs, and shallow fall is unforgiving of settlement, of a belly in a line and of the wrong pipe grade. A drain that works acceptably for twenty years can start holding water after very small ground movement, which is why a camera survey reads slope as well as blockage.
The housing stock spans an unusually wide range. Victorian and early stock through the central grid tends to sit on a raised floor with a crawlspace, which is a gift for diagnosis. Postwar and later tracts across the south and the Natomas basin moved to slab on grade with copper supply, and from that point the pipe went into concrete. A second slab leak in the same house within a couple of years is the signal that an overhead repipe has become the cheaper option rather than the expensive one.
Then there is metering. State law under AB 2572 required urban water suppliers to install meters on all customer connections by 1 January 2025 and to charge by volume. For a city with a long history of flat rate service that is a genuine behavioural change: a running toilet or a slow leak that used to cost nothing now shows up on a bill, and finding it is worth real money for the first time.

The areas below differ in the two ways that decide a Sacramento job: which drainage system they are on, and what era their housing comes from. The central grid is old housing on the combined sewer. Natomas, the Pocket and the southern tracts are newer housing on separated systems. Those are almost different trades.
Several places people think of as Sacramento are separate jurisdictions. West Sacramento is in Yolo County and runs its own water utility. Elk Grove, Rancho Cordova, Citrus Heights, Folsom and Galt are separate incorporated cities with their own building departments. And large unincorporated county areas including Arden-Arcade, Carmichael, Fair Oaks, Rosemont, Florin and North Highlands answer to Sacramento County rather than to the city.
That unincorporated belt is the most common source of confusion in this region, because a Sacramento mailing address is a poor guide to which agency issues permits or owns the sewer. Confirm from the address before anybody applies for anything.
One more practical note on the list above. The central grid neighbourhoods sit on smaller lots with shorter laterals but far older pipe, while Natomas, the Pocket and the southern tracts have newer pipe on longer runs across bigger lots. Neither is automatically the easier job. Old pipe fails predictably and is close to the kerb; new pipe fails rarely but when it does the excavation is longer and the access is worse.
Metering interacts with all of it. Once every connection is measured, the difference between a house with a hidden leak and a house without one shows up on a bill rather than staying invisible, and the first month a homeowner sees that number is very often the month they call. A meter test is the cheapest diagnostic in plumbing and in a metered city it is finally available to everybody.
